We Offer Dedicated and Personal Care Services
Professionals who are dedicated to improving the quality of life of individuals and their families.
Companion
Services will be provided to adult individuals (age 21 and older) when authorized.
Services will be based on the individual’s support plan to ensure that services provided follow a person-centered approach, the individual’s choices and outcomes, and are geared towards achieving the recipient’s goals.
Personal Supports
Personal Support will be provided to adults 21 years old and older living in the family home when authorized.
Goals will be obtained from the recipient’s support plan and an initial interview with the recipient.
Respite Care
Respite Care provides supportive care and supervision for individual under the age of 21 who live in the family home, when the primary caregiver is unable to care for the recipient due to a planned temporary absence, an emergency or when the primary caregiver is available, but unable to care for the recipient temporarily.
OUR GOAL
Support persons with disabilities in their daily living, learning, and working activities at home and in their communities to achieve maximum integration and self-sufficiency.
